PHN/CHN nurses in the community focus on protecting, promoting, preserving, and maintaining health while preventing disease. These goals reflect the ethical principles of promoting good and preventing harm. There are times there is a struggle and tension with the rights of local groups within a community and the individuals, families and groups you serve.

We serve populations that are varied on the socioeconomic strata, with access to resources, skills, support system, and access to health care services all being different. Fact: People with lower incomes and less education tend to be at a higher risk for health problems. However, the goal in the USA is to eliminate health disparities by expanding access to health care to vulunerable or "at-risk" populations.
